Why Colorectal Cancer Is Rising in Urban India

The same lifestyle forces driving obesity, diabetes, and cardiovascular disease in Indian cities are driving colorectal cancer: sedentary desk-bound work, diets high in refined carbohydrates and red meat and low in fibre and vegetables, rising alcohol consumption, smoking, and the demographic shift of India's urban population into the age bracket (over 45) where colorectal cancer risk rises meaningfully. The Indian gut microbiome — historically protective because of a traditionally high-fibre diet — is changing rapidly as dietary patterns Westernise. Colorectal cancer is the consequence.

How Colorectal Cancer Develops: The Polyp-Cancer Sequence

Over 95% of colorectal cancers begin as adenomatous polyps — benign growths on the inner lining of the colon. These polyps grow slowly and silently, without symptoms, over 10–15 years before undergoing malignant transformation. This extended precancerous window is why screening works so spectacularly: colonoscopy finds and removes the polyp before it ever becomes cancer. A colonoscopy that removes an adenomatous polyp has not just found a benign growth — it has prevented a cancer that would have developed years later.

Symptoms of Colorectal Cancer: The Problem of Late Presentation

Early colorectal cancer is asymptomatic. By the time symptoms appear, the cancer has usually grown to a size that has caused local complications. This is the central challenge: the symptoms that drive most Indian patients to seek medical attention — significant rectal bleeding, obstruction causing change in bowel habit, pain — are symptoms of established cancer, not early cancer. When symptoms do occur, the pattern depends on tumour location:

• Right-sided (ascending) colon cancer: Often no change in bowel habit (the right colon is wide-bore and liquid faeces can flow past the tumour easily). Presents with: iron deficiency anaemia (occult bleeding causing gradual anaemia), vague right lower abdominal pain or a palpable mass, and systemic symptoms (fatigue, weight loss). Right-sided cancers are typically detected later than left-sided ones.
• Left-sided (descending/sigmoid) colon cancer: Change in bowel habits — new constipation, ribbon-shaped stools, or alternating diarrhoea and constipation — as the narrowing lumen progressively obstructs solid faeces. Rectal bleeding (dark red or mixed with stool). Colicky lower abdominal pain.
• Rectal cancer: Fresh red blood in or on the stool (often mistaken for haemorrhoids), tenesmus (the constant urge to defaecate despite an empty rectum), a feeling of incomplete emptying, and pain with defaecation in locally advanced disease.

Rectal bleeding in anyone over 45, blood mixed with (rather than just coating) the stool, unexplained change in bowel habit lasting more than 3 weeks, progressive difficulty passing stool, or unexplained iron deficiency anaemia must be investigated with colonoscopy. Do not assume the bleeding is from haemorrhoids without proper examination and, where indicated, colonoscopy — this is the most dangerous diagnostic error in colorectal cancer in India.

• Family history of colorectal cancer or adenomatous polyps in a first-degree relative
• Personal history of colorectal polyps or colorectal cancer

• Hereditary syndromes — Lynch syndrome, FAP, MUTYH-associated polyposis
• Obesity and type 2 diabetes
