ERCP Procedure Explained: When You Need It for Bile Duct Stones

ERCP — Endoscopic Retrograde Cholangiopancreatography — is a procedure that sounds intimidating but is, for the right patient, one of the most precisely targeted treatments in modern gastroenterology. It is not a surgery. No incisions are made. A flexible camera is passed through the mouth, through the stomach, and into the first part of the small intestine (duodenum), where it accesses the opening of the bile duct. From there, stones can be removed, strictures dilated, and stents placed — all without a single cut on the skin.

What Is the Bile Duct and Why Do Stones End Up There?

The common bile duct (CBD) is the channel that carries bile from the liver and gallbladder to the small intestine. Gallstones that form in the gallbladder can migrate through the cystic duct and into the common bile duct. Once there, they cause problems: the bile duct becomes blocked, bile backs up into the liver, and jaundice develops. Infection of the bile duct (cholangitis) is the most dangerous complication — it is a medical emergency that requires urgent ERCP.

When Is ERCP Recommended?

ERCP is both diagnostic and therapeutic and is recommended when stones are confirmed in the bile duct. Main indications include:

• Jaundice with a dilated common bile duct on ultrasound
• Elevated liver function tests alongside gallstone disease
• MRCP confirming CBD stones before planned gallbladder surgery
• Acute cholangitis (fever, jaundice, upper abdominal pain)
• Pancreatitis caused by a gallstone blocking the pancreatic duct

Preparation

You will fast for 6–8 hours before the procedure. Blood thinners may need to be paused. Intravenous antibiotics are given before the procedure to prevent infection.

During the procedure

ERCP is performed under conscious sedation or general anaesthesia. The duodenoscope is passed through the mouth to the duodenum. A thin catheter is inserted into the bile duct opening. X-ray dye is injected and real-time fluoroscopy visualises the duct and locates the stone. A sphincterotomy is made and the stone extracted with a basket or balloon.

After the procedure

Most patients are discharged the same day or after an overnight stay. Mild throat discomfort and bloating are common and resolve quickly.

Risks of ERCP

• Post-ERCP pancreatitis: Occurs in 2–5% of cases. Usually mild.
• Bleeding: Usually self-limiting; significant bleeding is rare (less than 1%).
• Infection (cholangitis): Partially prevented by antibiotics.
• Perforation: Very rare, under 1% of cases.
