H. Pylori Infection in India: Symptoms, Testing and What Treatment Actually Involves

Helicobacter pylori (H. pylori) is the most prevalent bacterial infection in India — and one of the most consequential. Estimated prevalence in urban Indian adults is 50–70%, acquired in most cases in early childhood through contaminated water or food. H. pylori colonises the stomach lining, where it can persist for decades without causing obvious symptoms — but silently creating conditions for peptic ulcers, chronic gastritis, and ultimately stomach cancer. Understanding what H. pylori does, how to test for it accurately, and what eradication treatment involves is important for anyone with recurrent upper abdominal symptoms in India.

What H. Pylori Does in the Stomach

H. pylori is uniquely adapted to survive the hostile acid environment of the stomach. It produces urease — an enzyme that converts urea into ammonia, neutralising the acid immediately around the bacterium and allowing it to burrow into the mucus layer protecting the stomach lining. Once established, H. pylori triggers a chronic inflammatory response (gastritis) in the stomach lining. The consequences of this inflammation, over years to decades, include:

• Peptic ulcer disease: H. pylori disrupts the balance between acid production and the stomach's protective mucus layer, allowing acid to damage the stomach (gastric ulcer) or duodenal (duodenal ulcer) lining. H. pylori accounts for approximately 70–80% of peptic ulcers in India — the remaining are predominantly from NSAID use.
• Chronic atrophic gastritis: Long-term H. pylori infection leads to atrophy (thinning and loss of normal glandular cells) of the stomach lining — the precursor lesion for intestinal metaplasia and gastric cancer.
• Gastric cancer: H. pylori is classified as a Group 1 carcinogen by the World Health Organization — the only bacterium with this classification. The attributable risk of gastric cancer from H. pylori infection is approximately 75% globally. In India, with its high H. pylori prevalence and a diet with risk factors for gastric cancer (high salt, smoked and fermented foods in some regions, low fresh fruit and vegetable intake), this connection is clinically significant.
• MALT lymphoma: A rare gastric lymphoma that arises from H. pylori-stimulated lymphoid tissue in the stomach wall. H. pylori eradication alone can induce remission in early-stage gastric MALT lymphoma.

Does H. Pylori Always Cause Symptoms?

No — the majority of H. pylori-infected individuals are completely asymptomatic throughout their lives. The bacteria may cause chronic inflammation without producing pain or obvious symptoms. When symptoms do occur, they reflect the complications that H. pylori has produced — ulcers, gastritis — rather than the infection itself. Common symptoms in H. pylori-positive patients with peptic ulcer disease include: gnawing or burning upper abdominal pain, often worse on an empty stomach (duodenal ulcer pattern) or with meals (gastric ulcer pattern), nausea, bloating, and early satiety.

Testing for H. Pylori: Which Test Is Appropriate?

Several tests for H. pylori are available in India, with different accuracy profiles and clinical applications:

Urea Breath Test (UBT)

The gold standard non-invasive test. The patient ingests a small quantity of urea labelled with Carbon-13. If H. pylori is present, its urease enzyme breaks down the labelled urea and labelled CO2 is exhaled — measured with a breath analyser. Sensitivity and specificity over 95%. Best for: confirming eradication (must be performed at least 4 weeks after completing treatment). Requires stopping PPIs for at least 2 weeks and antibiotics for 4 weeks before testing for accurate results.

Stool Antigen Test (SAT)

A stool sample is tested for H. pylori antigens. Sensitivity and specificity comparable to the breath test. Convenient, non-invasive, and widely available in India. Also requires PPI cessation for 2 weeks before testing for accurate results. Good for both initial diagnosis and confirming eradication.

Endoscopic Biopsy (Rapid Urease Test + Histology)

During upper GI endoscopy, biopsies of the stomach antrum are taken. The rapid urease test (RUT) — placing the biopsy in a urea-containing medium and observing a colour change if H. pylori urease is present — provides a result within 30–60 minutes. Histology (microscopic examination of the biopsy) detects H. pylori and assesses the degree of gastritis and any mucosal changes. Best for: patients undergoing endoscopy anyway (for ulcer investigation, GERD assessment), where endoscopic biopsy adds no additional procedure.

Serology (Blood Antibody Test)

Blood tests for H. pylori IgG antibodies are widely available in India but are the least accurate for clinical use. A positive antibody test indicates exposure to H. pylori at some point — it cannot distinguish active current infection from past cleared infection. Serology should not be used to confirm eradication after treatment — antibodies persist for months after successful eradication.

H. Pylori Eradication Treatment

H. pylori eradication requires combination therapy — a single antibiotic does not work because H. pylori rapidly develops resistance. Standard regimens used in India include:

Standard triple therapy (14 days): A proton pump inhibitor (omeprazole, pantoprazole, or rabeprazole) twice daily + amoxicillin 1g twice daily + clarithromycin 500mg twice daily. Eradication rate approximately 70–80% — declining in areas with high clarithromycin resistance.

Bismuth quadruple therapy (14 days): PPI + bismuth subsalicylate + metronidazole + tetracycline — four drugs simultaneously. Eradication rates above 90%. Preferred in areas with high clarithromycin resistance, or as rescue therapy after failed triple therapy. Bismuth quadruple therapy is increasingly the preferred first-line in many Indian gastroenterology centres.

Completing the full course of treatment is non-negotiable. Partial completion is the primary driver of antibiotic resistance in H. pylori. Eradication should be confirmed 4 weeks after completing treatment with a urea breath test or stool antigen test — not serology.

Frequently Asked Questions

Should everyone in India be tested for H. pylori?

Not necessarily — universal population screening is not current practice in India, though it is debated given the high prevalence and gastric cancer risk. Testing is strongly recommended for: anyone with peptic ulcer disease (confirmed on endoscopy), anyone with unexplained upper abdominal symptoms that have not responded to standard antacid therapy, anyone with a family history of gastric cancer, and anyone with documented gastric cancer. A "test and treat" strategy (testing without endoscopy, then treating if positive) is appropriate in patients under 45 with dyspepsia without alarm symptoms.

Can H. pylori come back after eradication?

Reinfection after confirmed eradication is uncommon in adults in developed settings — approximately 1–2% per year in most studies. In India, where transmission routes (contaminated water, poor sanitation) persist, reinfection rates may be somewhat higher. Confirmed reinfection after successful eradication is treated with a second-line regimen using different antibiotics. Apparent "recurrence" on antibody testing usually reflects persistent antibodies from the original infection rather than true reinfection.

My doctor prescribed just one antibiotic for H. pylori. Is that correct?
