Can You Live With an Untreated Hernia? The Real Risks Explained

Many patients in Pune live with a known hernia for years — sometimes a decade or more. Some have been told to "watch and wait". Some are afraid of surgery. Some have had the hernia for so long that it feels like a normal part of life. The question this article answers honestly is: what actually happens to a hernia that is left untreated? The answer depends on the type of hernia, its size, what is inside it, and the individual patient. For some patients, watchful waiting is genuinely the right approach. For others, delay is accumulating risk. Here is the honest picture.

What Happens to Most Hernias Over Time: The Trajectory

The majority of hernias do not stay the same size. The defect in the abdominal wall — once present — is subject to the same mechanical forces every day: coughing, sneezing, straining at stool, lifting. These forces do not diminish over time. They progressively enlarge the defect. As the defect enlarges, more abdominal content protrudes, the hernia becomes more visible and symptomatic, and the anatomical situation for repair becomes more complex.

This enlargement is not universal — some hernias remain stable for years — but it is the most common trajectory. The patient who comes to Solo Clinic GI with an incisional hernia the size of a watermelon, a hernial sac that has permanently exteriorised a portion of their colon, who has had difficulty breathing for two years from the mechanical effect on their diaphragm — that patient had a small hernia ten years ago that they were told to watch.

The Three Complications of Untreated Hernias

Incarceration

The hernia content — bowel, omentum — slides into the sac and becomes stuck. The defect is too small to allow the content to return to the abdominal cavity. The result is a persistent, painful lump that does not reduce when lying down and does not go away. Incarceration is not immediately life-threatening if the bowel's blood supply is maintained — but it requires urgent surgical assessment. An incarcerated hernia that can be reduced (pushed back manually in the emergency department) may buy time for elective repair, but it signals that emergency repair may soon be needed.

Strangulation

Strangulation occurs when the blood supply to the incarcerated bowel is cut off by the pressure of the hernia neck. The bowel becomes ischaemic — it dies. Without emergency surgery within hours, the devitalised bowel perforates and bowel contents contaminate the abdominal cavity — causing peritonitis and sepsis, with a mortality rate that rises significantly with delay. Strangulation is the hernia emergency that kills. It is the outcome that a planned elective repair definitively prevents.

Obstruction

A hernia containing bowel can cause intestinal obstruction — the bowel cannot transmit its contents past the point where it is kinked or compressed within the hernia sac. Presents with colicky abdominal pain, vomiting, and inability to pass stool or flatus. Requires emergency assessment and often emergency surgery.

Which Hernias Carry the Highest Untreated Risk?

Femoral hernia: The highest strangulation risk of any hernia type — approximately 22% cumulative strangulation risk at 3 months. Femoral hernias should be repaired promptly after diagnosis regardless of symptoms. Watchful waiting is not appropriate.

Small-necked inguinal hernias: A hernia with a small, tight neck — where the defect is narrow but the sac protrudes — is at higher risk of strangulation than a hernia with a large, loose neck. Paradoxically, the most visible and "dramatic" hernias are often the safest, because a large neck allows free reduction of contents.

Incisional hernias with bowel content: Incisional hernias that contain bowel (rather than just omentum/fat) have higher strangulation risk and merit earlier repair.

Asymptomatic male inguinal hernias: Clinical trial data (the DELAY trial) shows that watchful waiting for asymptomatic male inguinal hernias is safe in the short term — the annual strangulation risk is approximately 0.18% per year. However, 70% of initially asymptomatic men become symptomatic within 7 years and ultimately require surgery — often with a more complex hernia than they started with.

Frequently Asked Questions

Is it safe to keep wearing a truss instead of having surgery?

A truss can prevent the hernia from protruding and provide symptomatic relief, but it does not repair the defect and does not prevent the long-term risks of incarceration or strangulation. A truss that is removed or shifts position leaves the hernia exposed. Long-term truss use is not a substitute for surgical repair in patients who are fit for surgery — it is a temporising measure for patients with very high surgical risk where the benefit of repair does not outweigh the risk of the operation.

Can a hernia fix itself?

No. An abdominal wall hernia — whether inguinal, umbilical, or incisional — does not close spontaneously in adults. The fascial defect that allows the hernia to form does not heal on its own once the hernia is established. (The exception is in infants, where umbilical hernias commonly close spontaneously by age 4–5.) Adults with established hernias should not expect spontaneous resolution.

My hernia goes away when I lie down. Is it still dangerous?

A reducible hernia — one that goes back into the abdomen when you lie flat — is generally lower risk than a non-reducible one. But "reducible" is not the same as "safe to ignore indefinitely". A hernia that reduces easily today may become incarcerated or strangulated tomorrow if a portion of bowel slides into the sac and gets trapped. The ability to reduce the hernia at rest does not guarantee it will always reduce. The reducibility of a hernia changes its urgency, not its eventual management requirement.
