Gallstone Pancreatitis: When Your Gallstones Attack the Pancreas

Gallstones are a common condition, and the vast majority of patients who have them — particularly those with silent stones — will never develop pancreatitis. But for the subset of patients in whom a small stone passes from the gallbladder, travels down the bile duct, and lodges temporarily at the ampulla of Vater (the shared opening of the bile duct and pancreatic duct into the duodenum) — pancreatitis is the consequence. Gallstone pancreatitis is the most common cause of acute pancreatitis in India, accounting for approximately 40–50% of all acute pancreatitis admissions.

How a Gallstone Causes Pancreatitis

The pancreatic duct and the common bile duct share a final common channel — the ampulla of Vater — before draining into the duodenum. When a small gallstone migrates from the gallbladder through the cystic duct into the common bile duct and passes (or gets stuck) at the ampulla, it creates a transient or sustained obstruction of both bile and pancreatic juice outflow. The backed-up pancreatic enzymes cannot drain normally into the duodenum, intraductal pressure rises, and this pressure — combined with potential bile reflux into the pancreatic duct — triggers premature activation of pancreatic enzymes within the gland itself. The result is acute pancreatitis.

Paradoxically, the stones most likely to cause pancreatitis are the smallest ones — tiny stones (microlithiasis) and gallbladder sludge (thick, partially settled bile) are more likely to slip through the cystic duct and into the bile duct than large stones, which are too big to migrate. A patient with multiple small stones or biliary sludge is at higher pancreatitis risk than one with a single large stone.

How Is Gallstone Pancreatitis Identified?

The blood test pattern distinguishes gallstone (biliary) pancreatitis from alcoholic pancreatitis: liver function tests typically show elevation of ALT and bilirubin in gallstone pancreatitis (reflecting the bile duct obstruction), while in alcoholic pancreatitis the LFTs may be less markedly deranged. An abdominal ultrasound showing gallstones (or a dilated bile duct suggesting recent stone passage) in the context of acute pancreatitis establishes the biliary cause. If the bile duct appears dilated or the LFTs suggest ongoing obstruction, MRCP confirms whether a stone is still present in the bile duct.

Treatment: Two Stages

Stage 1: Managing the Acute Pancreatitis

Exactly as for any acute pancreatitis — aggressive IV fluid resuscitation, analgesia, bowel rest, nutritional support. If there is concurrent cholangitis (bile duct infection, evidenced by fever, jaundice, and right upper abdominal pain together — Charcot's triad), urgent ERCP within 24 hours is performed to clear the bile duct and relieve the obstruction. In uncomplicated gallstone pancreatitis without cholangitis, ERCP is not required urgently — the stone often passes spontaneously.

Stage 2: Preventing Recurrence — Cholecystectomy

This is the most important and most frequently neglected step in India: laparoscopic cholecystectomy during the same hospital admission (ideally within 7 days of recovery from the pancreatitis), or within 2–4 weeks if early surgery is not possible. Without removing the gallbladder, the risk of another episode of gallstone pancreatitis is 30–50% within the following year — because the gallstones remain in the gallbladder and continue to be capable of migrating into the bile duct. Patients who are discharged after gallstone pancreatitis without a plan for cholecystectomy are at very high risk of readmission with another, potentially more severe attack.

Gallstone pancreatitis is not treated by treating the pancreatitis alone. The source of the problem — the gallbladder full of stones — must be removed to prevent recurrence. Frequently Asked Questions

My ultrasound shows gallstones but my amylase was only mildly elevated. Was this really gallstone pancreatitis?

Possibly — a mild self-resolving episode of gallstone pancreatitis can produce only modest amylase elevation, particularly if the stone passed quickly and the obstruction was brief. The pattern of LFT abnormalities (ALP, bilirubin elevation) alongside any amylase rise in the context of gallstones on ultrasound supports the diagnosis. Even a mild episode of suspected biliary pancreatitis is an indication for cholecystectomy — the "severity" of the first episode does not predict the severity of the next one, which could be much worse.

How soon after acute pancreatitis can I have gallbladder surgery?

For mild acute pancreatitis that has resolved clinically and biochemically, laparoscopic cholecystectomy can be safely performed during the same hospital admission — ideally within 5–7 days of presentation. Multiple randomised controlled trials have shown that same-admission cholecystectomy is safe after mild gallstone pancreatitis and reduces the risk of readmission before a delayed operation. For severe pancreatitis with necrosis or ongoing complications, surgery is deferred until the acute phase has fully resolved — typically 4–6 weeks or longer.

Can ERCP alone treat gallstone pancreatitis without surgery?

ERCP removes stones from the bile duct — it does not prevent new stones from migrating from the gallbladder. Without cholecystectomy, the source of bile duct stones (the gallbladder) remains. ERCP followed by cholecystectomy is the standard sequence for gallstone pancreatitis with bile duct stones. ERCP alone, without subsequent cholecystectomy, leaves the patient at ongoing risk of further biliary events. Cholecystectomy after ERCP is mandatory, not optional.
