Gallstones in India: Symptoms, Causes and When to See a Doctor

Most people in India who have gallstones don't know it. They go years — sometimes decades — with stones silently sitting in the gallbladder, causing nothing. But when those stones start to cause trouble, they cause it decisively: a severe pain in the upper right abdomen that can floor you within minutes of a meal. Understanding what gallstones are, why Indians are increasingly susceptible to them, and when to actually seek medical help can save you from an unnecessary emergency admission.

What Are Gallstones?

The gallbladder is a small, pear-shaped organ under the liver. Its job is to store bile — a digestive fluid the liver produces — and release it when you eat, particularly fatty foods. Gallstones form when the chemical composition of that bile becomes imbalanced, causing solid deposits to crystallise inside the gallbladder.

There are two main types. Cholesterol gallstones — the most common in India — form when bile contains too much cholesterol. The excess crystallises into pale yellow or greenish stones. Pigment gallstones are darker, made of bilirubin, and more common in people with conditions that cause rapid red blood cell breakdown, like sickle cell anaemia or haemolytic disorders. Most Indians who develop symptomatic gallstones have cholesterol-type stones.

Why Are Gallstones Common in India?

India's changing dietary patterns and lifestyle are driving rising gallstone rates, particularly in urban populations. The key risk factors are more prevalent than ever:

• High refined carbohydrate intake: White rice, maida-based foods, and sugar-heavy diets elevate cholesterol in bile
• Obesity and metabolic syndrome: The liver overproduces cholesterol in people who are overweight, supersaturating bile
• Rapid weight loss: Crash diets and bariatric surgery cause the liver to secrete extra cholesterol into bile and reduce gallbladder emptying — a recognised trigger
• Pregnancy: Oestrogen increases cholesterol secretion; progesterone slows gallbladder emptying. Multiple pregnancies multiply the risk
• Diabetes and insulin resistance: Associated with impaired gallbladder motility and increased biliary cholesterol
• Sedentary lifestyle: Physical activity increases gallbladder motility; inactivity allows bile to stagnate
• Family history: A first-degree relative with gallstones raises your risk two to three times

Gallstone Symptoms: What They Actually Feel Like

The majority of gallstones — perhaps 70–80% — are entirely silent and never cause trouble. The problem begins when a stone migrates toward the narrow neck of the gallbladder or the cystic duct, blocking bile flow. This is called biliary colic.

Biliary colic feels like this: a sudden, severe cramp-like pain in the upper right abdomen or just below the breastbone (epigastrium), often radiating to the right shoulder blade or mid-back. It typically begins 30 minutes to 2 hours after a meal — especially a fatty or heavy one. The pain builds rapidly to a plateau, stays there for 30 minutes to several hours, then subsides. Between attacks, you feel completely fine. This wave-like pattern, tied to meals, is the classic signature of gallstone disease.

Other symptoms that accompany or develop alongside biliary colic include nausea and vomiting, bloating and belching (often dismissed as "gas"), intolerance to oily or fried food, and a feeling of fullness or indigestion after meals. If your upper abdominal pain is constant (not wave-like), accompanied by fever and chills, or if your eyes or skin are turning yellow — these suggest an infected gallbladder or blocked bile duct. Both are urgent. Go directly to the nearest emergency department.

Silent Gallstones vs Symptomatic Gallstones

The distinction matters enormously for treatment decisions. Silent gallstones — found incidentally on an ultrasound done for another reason — do not require surgery in most adults. The annual risk of developing symptoms from a silent gallstone is approximately 1–2%, and the risk of serious complications without a prior symptomatic episode is low. Watchful waiting, with an understanding of what symptoms to look out for, is a reasonable approach.

Symptomatic gallstones are a different conversation. Once you have had a biliary colic episode, the risk of a repeat attack or a complication (infected gallbladder, bile duct stone, or pancreatitis) is significant over the following year. Surgery is generally recommended after the first clearly symptomatic episode.

How Gallstones Are Diagnosed

Abdominal ultrasound is the gold standard first investigation for suspected gallstone disease. It is accurate, non-invasive, and inexpensive. An ultrasound will show the stones (their size, number, and location), the thickness of the gallbladder wall (suggesting inflammation), and the diameter of the bile duct (widening suggests a stone may have passed into the duct). Blood tests — liver function tests, bilirubin, and inflammatory markers — complement the ultrasound by showing whether the liver or bile duct is affected.

Frequently Asked Questions

Can gallstones go away on their own?

No. Formed gallstones do not dissolve or disappear without treatment. Dietary changes can reduce the frequency of attacks but do not remove the stones. Some very small stones can occasionally pass through the bile duct spontaneously, but this is rare and can itself cause a dangerous blockage. Once symptoms begin, surgery is almost always recommended.

Does a high-fat diet cause gallstones?

Not directly. Gallstones form when cholesterol in bile becomes supersaturated — which is more strongly associated with total caloric excess, refined carbohydrate intake, obesity, and rapid weight loss than with dietary fat alone. Paradoxically, a very low-fat diet can worsen stone formation by reducing gallbladder emptying (the gallbladder needs fat in the meal to squeeze out bile). A balanced, moderate-fat diet with high fibre is healthiest for gallbladder function.

Can I have gallstones without any pain?

Yes — the majority of gallstones are entirely asymptomatic. They are often found incidentally on ultrasounds done for unrelated reasons. Silent gallstones generally do not require surgery. Your doctor will discuss whether your specific stone characteristics (size, number, gallbladder wall thickness) place you in a higher-risk group that warrants closer monitoring.

How quickly should I see a doctor after a gallstone attack?

Within a week for a first attack if pain has fully resolved and you have no fever or jaundice. Immediately — via emergency — if the pain is constant and severe, you have fever and rigors, your eyes or skin are yellow, or you are vomiting persistently. A first attack that has settled can be investigated electively, but delay beyond a few weeks increases the risk of a second, potentially more serious episode.

Is there a non-surgical way to treat gallstones in India?

For symptomatic gallstones, surgery (laparoscopic cholecystectomy) is the definitive treatment. There is no medication that reliably dissolves cholesterol gallstones in a clinically meaningful timeframe. Ursodeoxycholic acid (UDCA) can slowly dissolve very small, pure cholesterol stones in a non-functioning gallbladder — but this applies to a very narrow set of patients and takes 6–18 months with a high recurrence rate once stopped. The vast majority of patients with symptomatic gallstones are better served by laparoscopic surgery.
